Caso clínico: Úlcera vascular venosa en un paciente con disminución intelectual

Resumen: Paciente de 68 años de edad, con discapacidad intelectual, institucionalizado, y con un grado de dependencia importante para la realización de las AVD.
Presenta una úlcera vascular venosa que se complica con la aparición de nuevas úlceras y una celulitis a los pocos días del inicio  del tratamiento. 

Metodología: Corregimos  satisfactoriamente la celulitis mediante antibiótico terapia vía oral  y las úlceras con la utilización de apósitos cada 48 horas para modificar en la menor medida posible la rutina del paciente, dadas sus características y preferencias.
Para tal fin, se utilizaron apósitos no adhesivos hidropoliméricos y con plata iónica hidroactiva.
Uno de los principales problemas que se nos planteaba era cómo realizábamos la sujeción de dichos apósitos para no dañar la zona perilesional y teniendo en cuenta, además del estado de la lesión, las características del paciente: disminución psíquica,  nula conciencia de enfermedad y merma importante en su autonomía; además de presentar una alteración en la percepción del dolor así como dificultades para expresarlo.
Basándonos en nuestra intuición y quehacer profesional pensamos que lo mejor, en este caso, era la utilización de medias de compresión decreciente favoreciendo también la circulación y retorno venoso.

Conclusiones: El resultado: la correcta curación de la herida en un tiempo relativamente corto y  sin que ello representará para el paciente ningún cambio ni molestias importantes en su vida diaria.

Title: Clinical case: Venous vascular ulcer of a patient with intellectual decrease

Summary: Here is presented a clinic case of a patient who is 68 years old, who suffers a intellectual discapacity, instituzionalized, and with a serious dependence degree for the realization of the AVD, he presents also a venous vascular ulcer that is getting worst because of the apparition of new ulcers and a cellulites during the first days the beginning of the treatment.

Methodology: Apart from correcting satisfactorily the cellulites with the biotherapy cures every 48 hours, with the main objective of modi-fying in the lower way the routine of the patient, for her characteristics and preferences; but always having in mind the effectivity in the achievement of a correct cutaneous integrity. For this method, hidropolimérics gauzes non adhesive with hidroactiv ionic silver were used. One of the main troubles that we had war how to get the correct subjection of there gauzes, to not damage the perilesional zone and having on count such as things as the state of the injury and the characteristics of the patient; psy-chic decrease, without conscience of his illness and important reduction in his autonomy, as well he presents an alteration for the perception of pain, he also has difficulties in expressing it. With our pro-fessional experience and intuition we thought that the best for this care, war the use of deacrising compression bandages for the subjection of there gauzes, the deacrising compression stocking also helps the blood circulation and the venous return.

Conclusions: The result of thin process is the cor-rect and absolute recovery of the injury in a few time, without a change or important incomoditier in the daily life of the patient.
