Cas clínic : Úlcera vascular venosa d’un pacient amb disminució intel·lectual

Resum: Es presenta un cas clínic d’un pacient de 68 anys d’edat, amb una discapacitat intel·lectual, institucionalitzat, i amb un grau de dependència important per a la realització de les AVD. Presenta una úlcera vascular venosa que es complica amb l’aparició de noves nafres i una cel·lulitis al cap d’uns dies de l'inici del tractament.
Metodologia: A part de corregir correctament la cel·lulitis amb tractament antibiòtic, les úlceres es van tractar amb la utilització d’apòsits que van permetre espaiar les cures cada 48 hores, amb l’ob-jectiu de modificar el mínim possible la rutina del pacient, donades les seves característiques i prefe-rències. Es van utilitzar apòsits no adhesius hidropolimerics i amb plata iònica hidroactiva.
Un dels principals problemes que se'ns van plantejar era com realitzaríem la subjecció d’aquests apò-sits per tal de no danyar la zona perilesional, tenint en compte, a més l’estat de la seva lesió, les ca-racterístiques del pacient: disminució psíquica, nul·la consciència de malaltia i dificultat important en la seva autonomia; a més a més de presentar alteracions en la percepció del dolor així com dificultats per expressar-ho. Basant-nos en la nostra intuïció i experiència professional vam pensar que el mi-llor, en aquest cas, era la utilització de mitges de compressió decreixent afavoreixen la circulació i el retorn venós. Conclusions: El resultat: la correcta curació de la ferida en un temps relativament curt i sense que això representes per al pacient cap canvi ni molèsties importants en la seva vida diària.

Title: Clinical case: Venous vascular ulcer of a patient with intellectual discapacity

Summary: Here is presented a clinic case of a patient who is 68 years old, who suffers a intellectual discapacity, instituzionalized, and with a serious dependence degree for the realization of the AVD, he presents also a venous vascular ulcer that is getting worst because of the apparition of new ulcers and a cellulites during the first days the beginning of the treatment. Methodology: Apart from correcting satisfactorily the cellulites with the biotherapy cures every 48 hours, with the main objective of modi-fying in the lower way the routine of the patient, for her characteristics and preferences; but always having in mind the effectivity in the achievement of a correct cutaneous integrity. For this method, hidropolimérics gauzes non adhesive with hidroactiv ionic silver were used. One of the main troubles that we had war how to get the correct subjection of there gauzes, to not damage the perilesional zone and having on count such as things as the state of the injury and the characteristics of the patient; psy-chic decrease, without conscience of his illness and important reduction in his autonomy, as well he presents an alteration for the perception of pain, he also has difficulties in expressing it. With our pro-fessional experience and intuition we thought that the best for this care, war the use of deacrising compression bandages for the subjection of there gauzes, the deacrising compression stocking also helps the blood circulation and the venous return. Conclusions: The result of thin process is the cor-rect and absolute recovery of the injury in a few time, without a change or important incomoditier in the daily life of the patient.
